Can I dispute an item on my credit report without having a copy of my credit report?
I think you have to first get your report, then use the number code on it in your letter to refer to your report so they make money that you ordered the report first before considering your dispute, but it says I already got my free annual report (which I didn’t) and now have to pay for one. I know for 100% certainty of a particular debt that has been reported to the credit bureaus based on a letter from the creditor. Can I just write to them to request they remove it without purchasing their report first?

No, you don’t need to purchase a credit report to dispute errors. That is not a factor in whether the credit bureaus process your dispute. You can send a letter, include copies of your driver’s license, social security card, and a utility bill at your current address. Just list the creditor and why you are disputing.
